- Call your Enhanced Benefits Group representative at 866-505-3244 Our knowledgeable customer service representatives are available to serve you Monday through Friday 9:00 AM until 5:00 PM. They will explain how the program works, identify what your needs are and connect you with the appropriate service provider.
- Quick and Secure online Application. Click here to get started Input all of the fields within the application and push SUBMIT. This application will go directly to your Enhanced Benefits Group customer service representative. Within 24 hours, you will receive a phone call from the representative to confirm receipt of the application and to walk you through the next steps based on your need.
- Fax your application to Enhanced Benefits Group at 877-472-2770
Download the PDF. Fill in all of the fields on the application and print it from the PDF file before faxing it to 877-472-2770. A customer service representative will call you by the next business day to confirm receipt and answer any questions that you have.
